The Lutheran Services in America network is deeply embedded in the fabric of communities across the country.

More than 200 organizations in our network provide a broad range of services to older adults in 1,400 communities across 46 states. We are among the largest non-profit providers of post-acute care and senior residential care in the United States, offering affordable housing, independent living, assisted living and skilled nursing. Our network is a trusted provider of community-based services including home healthcare, senior centers, care coordination, transportation, behavioral health, respite care and hospice care among other services.

Working with people in their home and addressing unmet social determinants of health is core to our role as nonprofit providers of service.

Lutheran Services in America organizations are recognized for their holistic approach to service and addressing the needs of the whole-person. This work includes, for example, connecting older adults to services in the community that improve their quality of life, providing caregiver support and offering assistance with food, housing and transportation among others.

Leveraging our network’s deep knowledge and experience addressing the social determinants of health in older adults.

In 2019, we engaged thirteen leading providers of care from our network and formed a collaborative with the goal of integrating best practices from the field to create LSA Senior Connect.


LSA Senior Connect screens older adults for risks tied to social determinants of health and connects them to services and supports in their community. And, it enables older adults with chronic health conditions who live in their home or low-income housing to stay healthy, avoid hospitalization and continue to live independently in the home and community of their choice.

National results, local impact.

LSA Senior Connect is a service coordination solution that trains staff to conduct comprehensive screenings of residents and their social determinants of health, including home safety, nutrition and access to transportation, and connects them to services in the community. The model relies on a compilation of evidence-based practices to address the needs of the individual holistically.

LSA Senior Connect addressed gaps in care among  1,160 affordable housing residents in Toledo, Ohio

Promising results.

Genacross Lutheran Services implemented LSA Senior Connect in its affordable housing program and uncovered 1,526 needs, including socialization, healthy food, reliable transportation to medical appointments, in-home supports and physical therapy. Resident coordinators addressed most of these needs by connecting older adults to community resources.
